SUSANNAH KERWIN, HNP/ANP-BC, FMCP-M
Functional Medicine Clinician
Susannah Kerwin, HNP/ANP-BC, FMCP-M is a board-certified nurse practitioner and functional medicine clinician with over two decades of experience helping patients reconnect with their health through a personalized, root-cause approach to care. Her work is grounded in the belief that complex, chronic conditions require time, nuance, and a level of attention that extends far beyond conventional, one-size-fits-all medicine.
Emergence Health is honored to welcome Susannah to the practice in support of maternity leave coverage for Leah in 2026. Leah and Susannah have been colleagues for over a decade, as well as trusted peers and friends. Having also experienced care as a patient of Emergence Health, Susannah brings a rare, firsthand understanding of our practice and philosophy.
Susannah specializes in perimenopause and menopausal hormone health (including HRT), weight loss resistance, thyroid health, gut health, and mold detox. She is the founder of Tendwell: a Colorado-based, functional medicine and primary care practice. Her work focuses not only on resolving symptoms, but on restoring clarity, resilience, and long-term balance in health.
Susannah takes the time to understand each patient’s full picture - medical history, environment, lifestyle, and lived experience - before shaping a clear and structured plan forward. She creates a process in which patients are consistently engaged, informed, and supported, allowing for meaningful progress that is both sustainable and aligned with their lives.
Earlier in her career, Susannah lived and practiced in San Francisco and New York City, and spent time abroad in Beijing, where she explored traditional Chinese medicine. These experiences broadened her perspective on healing and reinforced her understanding of the limitations of conventional, fast-paced healthcare models - particularly for those navigating more complex or persistent health concerns.
Susannah earned her Bachelor of Science in Nursing from the University of San Francisco, where she developed a strong clinical foundation in patient-centered care. She went on to complete her Master of Science in Nursing at New York University, specializing in adult health, further advancing her ability to care for patients with complex medical needs. She is also a functional medicine certified practitioner (FMCP-M) through the Institute for Functional Medicine, where her training emphasizes systems-based, root-cause methodologies that integrate conventional and integrative approaches to care.
Outside of her work with patients, she enjoys spending time in Colorado nature with her children and husband, reading, traveling, exploring new places, playing soccer and practicing hot yoga.
