HERE'S WHAT'S INCLUDED
Holistic Physical Exam (60 mins)
The most thorough physical exam you will ever receive. During this 60-minute visit, we review your concerns, health history, family history and current lifestyle routines around sleep, stress management, relationships, diet, and exercise. Our goal with this visit is to get to know you and identify areas to optimize your wellness and screen/ prevent disease.
Initial Follow-Up Visit (60 mins)
Your first follow-up visit is one hour, during which your medical provider combines your medical history, personal preferences, and test results together to create a personalized health plan designed to identify and treat your health issues at the root cause.
3 Follow-Up Visits (30 mins)
Each of your 3 follow-up visits are 30-minutes long and with the same provider who did your Holistic Physical Exam and Initial Follow-Up Visit. These visits are designed to re-evaluate the initial individualized plan, track results, provide in-depth analysis, and optimize treatment recommendations based on current progress.
Routine Lab Reviews
Routine labs are typically run through insurance and do not require cross-discipline collaboration with lab specialist(s) for review. These include, but are not limited to, the following labs: Quest, LabCorp and Northwell Health. Your cost responsibility for these tests is contingent upon your insurance plan and do not require cross-discipline collaboration with lab specialist(s) for review.
Personal Wellness Plan
Plans are tailored specifically for your needs and goals. They cover diagnostic testing, nutrition, fitness, mental health, medications, medical-grade supplement recommendations, and more.

1. SCHEDULE AN APPOINTMENT BY PHONE OR ONLINE.
If you become a member of our office, you will receive access to our online patient portal which you can use to schedule appointments. You can also call or text our office number for scheduling. These services are HIPAA compliant, so your private health info is safe. The office will send you an email confirmation once your visit is approved.
INSURANCE-EVERYTHING YOU NEED TO KNOW.
Each visit and unlimited messaging are covered by your Emergence Health membership fee. Some of this fee may be reimbursed by your insurance company. We also encourage our members to use insurance for costs outside of membership including lab tests, prescriptions, and supplements.

FREQUENTLY ASKED QUESTIONS
WHAT IS NOT INCLUDED IN MY PLAN?
Cost of laboratory tests and imaging, though many tests are covered partially or fully by insurance depending on your plan. Healthcare services beyond those specified above including home visits, specialty labs, and specialty lab reviews. These services are optional and can be coupled with one of your membership visits for an additional fee. Medications (over-the-counter or prescription) and supplements are covered to the degree of your insurance. Non-medical services recommended by Emergence Health but provided by others, such as health coaches, acupuncturists, or food services.
CAN I USE FSA, HSA OR INSURANCE?
We accept FSA/HSA plans as payment for your membership. We do not accept insurance; however, membership fees can be submitted to your insurance for out-of-network reimbursement up to 70% depending upon your plan. We make submitting out-of-network claims easy through our partner, Reimbursify.
WHAT IF I CHANGE MY MIND ABOUT MEMBERSHIP?
All membership plans offered through the practice are for an annual, 12-month, commitment. You have 30 days from purchasing your membership plan to change your mind. If you have not used any of your membership plan, you will be issued a full refund. If you have used components of your membership plan/benefits, you will receive a full refund minus fees associated with any services used according to the following:
- $150 per 15 minutes (ex. 30-min visit=$300)
Renewal is automatic. You may cancel your membership plan renewal within 30 days of the renewal date.
DOES MY MEMBERSHIP INCLUDE TESTING?
We work with you to customize testing based on your health needs, personal preferences, and finances. Costs for blood work and specialty tests are not included in the membership price. Blood work is usually covered by insurance. Specialty tests range in cost from $100 - $425 and are optional. Fees are subject to what your insurance will cover. For certain specialty tests, we also require a lab consultation fee, which will be described in detail on the consent form for the test.
